Service Information and Life Skills Facilitator
Part-time, 10 hours per week
The CommUNITY team is passionate about working with communities to provide opportunities for individuals to be empowered, connected and achieving. The CommUNITY service plan specialised services and supports that culturally and linguistically meet client needs through Community Access, an information, referral, advocacy, and awareness service. The service also delivers resources through community information workshops, and seeks to understand the aspirations of the community through engagement events.
The Lifestyle Support Service (LSS) assists adults under the age of 65 with and without disabilities, and who primarily use Auslan to communicate. LSS provides Deaf and hard of hearing participants with access to one on one life skills development, helping them learn and develop the skills and the confidence to live independent lives. Life skills development is available as a fee for service, or as part of an NDIS package.
The Service Information and Life Skills Facilitator will be pro-active in enabling Deaf Services to respond to the service requirements of Deaf and hard of hearing individuals through the provision of Community Access – an information and referral service. The position is also responsible for working in partnership with Deaf and hard of hearing individuals with a NDIS package to enable them to gain skills and knowledge to live independently through facilitation and provision of learning opportunities.
The Service Information and Life Skills Facilitator is a member of the CommUNITY and Lifestyle Support Services teams and works collaboratively with others to ensure the effective administration of systems and processes that support Community Access, Life Skills and other community programs and services with a focus on clients, quality and continuous improvement.
